For leaders

Walk report / guest waiver form to print PDF version (the usual)

.doc version (editable). This could be useful for a contact leader walk.

Please type up and email your report to Jacqui. Address is at the bottom of the form.

It would help the reports managers if you type the list of walkers along a line separated by comma and space and the optional comments for the newsletter in the body of your email.
